This is a complaint form used to report workplaces violating labor relations laws, such as unpaid wages or workplace harassment, to the Ministry of Employment and Labor. Workers can apply if they did not receive wages on the regular payment date while employed, or if they did not receive wages, severance pay, allowances, etc. within 14 days after resignation.
