This is a form used by construction workers who have worked at construction sites to claim retirement mutual aid funds. Workers with 252 or more accumulated days of retirement mutual aid can apply if they have retired from the construction industry or reached the age of 60, or if they have less than 252 accumulated days but have reached the age of 65. If the worker has passed away, a bereaved family member can claim on their behalf. Answering the questions will generate the claim form as a PDF.
